Life Insurance Life insurance coverage is provided at one times your salary. Accidental Death and Dismemberment coverage is also provided at one times salary. An employee’s spouse is insured for $2,000 and each dependent child is insured for $1,000. This is an employer paid benefit. Voluntary Life Insurance Voluntary Life insurance coverage is offered to our employees and is subject to a medical questionnaire at the time of enrollment. We also offer the life insurance to our employee’s spouse and all dependent children at cost to the employee. It is based on age and different costs apply to each applicant. It is offered after-tax and you may enroll or drop at the end of the payroll month during the year. This is strictly a voluntary benefit offered to our employees. |
