What is a Food voucher?
Called Wake Assistance, it is a grant from the Menominee Tribe that went into effect on October 23 1995. It is to assist our deceased enrolled Menominee Tribal members and their families with a voucher for food to help cover some costs.

Who is eligible?
Deceased enrolled Menominee Tribal members, family members of the deceased can apply and receive the food voucher for the funeral meal.

How do I know if my family member is eligible?
The Insurance Dept. has an online system connect with the Menominee Tribal Enrollment, that provides us with up to date enrolled members listed. Or you can contact the Menominee Tribal Enrollment Dept. to become enrolled.

How much is given?
Up to $125.00, anything over, is the deceased family member’s responsibility.

When and how long does it take to receive this Food Voucher?
Contact the Insurance Dept. as soon as you can, the sooner the better. We must follow our policies to provide your family with the proper entitlement that your loved one deserves. It may take as much as a few hours to possibly the next day, depending on the time you contact the Insurance Dept.

Who do I contact for this benefit?
You must contact the Menominee Tribal Insurance Dept at (715) 799-5185 or 5188.

What information do I need for the Insurance Dept.?
The Menominee Tribal Insurance Dept. understands you may not have all the information, but it would be helpful to have:

  1. Correct Name or nickname.
  2. Correct Date of Birth.
  3. Correct Date of Death.
  4. What Funeral home will be handling the Funeral arrangement?
  5. Who will be preparing the meal for the service, their name will be on the voucher to pick up the food?
  6. What grocery store would you like to pick up the food from?
    Choices are, Pick & Save or Country Store
•All other information needed, the Insurance Dept can assist you in this matter.

Please, after you are done purchasing your food from the store of your choice, you must return a copy of your grocery receipts to the Insurance Dept. So this bill can be paid officially.
