The following benefits information applies to full-time, regular employees. For other temporary, part-time, contract, or on-call employees, contact the HR department. (Please see your various plan documents for a complete description of coverage.)
  1. Health Insurance
    The Menominee Indian Tribe has a partially self-funded group medical and short-term disability plan. The third party administrator is Wausau Benefits. The plan contains three parts: Tier 1, Tier 2 and Tier 3.
    1. Tier 1 - MITW (Clinic)
      $100 Deductible/Individual
      $200 Deductible/Family
      80% Co-Insurance with an out of pocket maximum of $500/Individual and $1000/Family.

    2. Tier 2 - PPO (WPPN)
      $250 Deductible/Individual
      $500 Deductible/Family
      80% Co-Insurance with an out of pocket maximum of $1050/Individual and $2100/Family.

    3. Tier 3 - Non-PPO
      $500 Deductible/Individual
      $1000 Deductible/Family
      70% Co-Insurance with an out of pocket maximum of $2000/Individual and $4000/Family.

    Most charges are subject to deductible and 80% except for Wellness Benefits which are paid at 100%.

  2. Dental coverage
    It pays 100% payment for checkups and cleanings and 80% for repair work, root canals, periodontal services and oral surgery. Maximum benefit per person per year is $1200. There is 50% coverage for bridges, crowns or dentures. There is a $2,000 lifetime maximum for orthodontia for dependent children only, which is paid at 50% up to $750 per year. No deductible. Delta Dental is the third party administrator for dental coverage.

  3. Vision care coverage
    Includes; contact lenses and eye glasses payable at 50%, deductible waived, to a maximum benefit paid of $150 per Covered Person, per Calendar Year, on a Reasonable and Customary basis.

  4. Prescription drugs
    Are paid for through a prescription drug program administered by Merck-Medco. There is a $5 CO-payment for generic drugs and a $20 CO-payment for brand name drugs. Mail order prescription program is $10 per generic and $45 per brand name.
Coverage is effective the first day of the month following the completion of the 60th day of employment. The employee contribution is $96 per paycheck for family coverage and $48 per paycheck for single coverage. A Section 125 Premium Only Plan has been established that allows employees to pay for their health insurance and certain voluntary insurance premiums with pretax dollars.
  1. Short-Term Disability
    This coverage will provide you with 67% of your weekly wage during periods of disability. Benefit payments begin on the first day of disability if disability is due to an accidental injury, and on the eleventh day of disability if due to an illness. The maximum benefit period is 26 weeks. Wausau Benefits is the third party plan administrator for Short Term Disability insurance.

  2. Life Insurance
    Life insurance coverage is provided at one times salary. Accidental Death and Dismemberment coverage is also provided at one times salary. An employee's spouse is insured for $2,000 and each dependent child is insured for $1,000.

  3. 401k Retirement Savings Plan
    The 401k is a separate enrollment process. Employees are eligible for the Tribe's 401K plan upon attainment of age 21 and completion of 6 months of service. Enrollment in the Plan occurs April 1 and October 1 of each year. Contributions are based on a percentage of your salary and you may contribute up to 16% of your salary. After one year, the Tribe will match your contribution up to 4% of your salary. Investment of contributions may be made in any of the ten- (10) available investment funds.
