Burial Insurance Benefit
 
The Menominee Indian Tribe of Wisconsin provides a Burial Benefit to enrolled Tribal Members. This policy provides the following benefits:
  • For tribal members 6 months or older, up to $3,000.
  • For tribal members 0-6 months of age, up to $300.
Tribal members 6 months or older must be enrolled members to be eligible for this benefit.

For children 0-6 months of age to be eligible for the benefit, all the following conditions must be met:
  • One of the parents is an enrolled tribal member.
  • Had an application for enrollment submitted to the Enrollment Office prior to death.
  • Tribal membership for the child is subsequently approved.
A copy of the birth certificate will be needed when applying for the benefit. Claims Procedure
A certified copy of the death certificate and a copy of the funeral home bill must be sent to the Insurance Department in order for a claim to be filed. If the decedent is 0-6 months of age, a copy of the birth certificate must also be submitted with the death certificate and funeral home bill.

The benefit is paid directly to the funeral home and not to any one individual. The person who signs the funeral home bill will be the contact person for the Insurance Department. To take full advantage of this benefit, it is suggested that this benefit be used first before any other available resources. Payment to the funeral home will be made 30-45 days after the claim is filed. The following examples illustrates how the benefit works:
  • Example 1.
    If the funeral home bill is $2,950, the Tribe will pay the funeral home $2,950. The remaining $50 will be used to offset the future costs of the program. It will not be paid to any individual or the funeral home.
  • Example 2.
    If the funeral home bill is $6,100, the Tribe will pay $3,000 to the funeral home. The balance of $3,100 will be the responsibility of the family.
