Tax Exempt Card Application

Please Read This Notice Carefully

There have been questions which have arisen since the first issuance of the I.D. Cards. Please read the following stipulations as to the use of the card. The issuing of this card is for use as an identification card by enrolled Menominee's to prove they are indeed an enrolled member of the Menominee Indian Tribe. The number issued is a "certified enrollment number", for use in applying for various programs within the tribal government. This card shows that you are an enrolled member of the Menominee Indian Tribe for delivery of goods or services to the reservation by a retailer, for use of that goods or services on the reservation. This type of delivery is DEFINITELY state tax exempt. If you have any questions regarding the Tax Exempt Card, please call the Licensing and Permits Department at (715)799-5142.
